Insurance
I am currently an out-of-network provider, which means I’m not in-network with any insurance companies and payment is due after each session. If you have out-of-network benefits (with Premera, Aetna, or Cigna for example), I can provide you with a superbill that you can submit to your insurance company, which may cover all or part of your session fee. Please check with your insurance company to verify if you have out-of-network benefits and see the suggested questions below to aid in your request for information. Please note: I am a private-pay only provider for couples therapy.
Things to consider when using health insurance:
When using insurance, coverage is based on the rules of that insurance company. This means that the insurance company could determine the length of our appointments, type of therapy covered, and number of appointments that they would cover. In addition, it may be a requirement to provide a mental health diagnosis (such as Major Depressive Disorder) when billing insurance, which then becomes part of your medical record.
When paying for therapy out-of-pocket, there is no disclosure of treatment, length of appointments, or diagnosis required. There is no third party involvement, and your treatment remains private unless you choose to disclose that information.
If you plan to use insurance, please be sure to check your insurance benefits before beginning therapy.
Some questions to ask your insurance provider:
Do I have out-of-network insurance benefits for mental health?
Are Licensed Mental Health Counselor Associates (LMHCA) covered by out-of-network benefits?
What is my deductible? How much has been met so far?
How many sessions per year are covered?
What amount is reimbursed per therapy session for an out-of-network provider?
Do I need approval from my primary care doctor?
What forms do I need to file? How do I file these forms?
If it’s helpful, you can provide the following CPT codes to your insurance company:
90791 for individual intake
90834 for 45-minute individual psychotherapy sessions
90837 for 60-minute individual psychotherapy sessions
